Baptism Sign-ups

We are grateful that you and/or a family member are considering baptism at CPC. This form is for requesting Baptism, Reaffirmation of Baptism, or Baby Dedication.

Baptism: Baptism is a sacrament of the church, a public proclamation of your personal decision to follow Jesus. It's a symbol of the death, burial, and resurrection of Christ and the celebration of that power in our life. It is received only once. Parents may present their child for baptism up to the age of 4. For kids, we recommend waiting until the age of 7.

Reaffirmation of Baptism: If you have been baptized before (as an infant or child) and would like to reaffirm your baptism, you have the choice of being immersed or sprinkled with water.

Baby Dedication: A dedication is a blessing/prayer over babies younger than 2, in which Christian parent(s) make a commitment before the Lord to submit a child to God's will and to raise that child according to God's Word and God's ways.
